GitHub allows not only to create issue templates, but also issue forms.

We implement an issue form template here that can be reused accross repositories.

Describe alternatives you've considered

Sticking with issue templates leaves third parties with markdown templates they might not easily comprehend.

This does not exist for PR templates.

For inspiration how this could look like, I have recently aggregated issue templates for our internal issue tracker.

In doing so, I came across the fabulous example at https://github.com/appium/appium/blob/master/.github/ISSUE_TEMPLATE/bug_report.yml
