Add the PD feedback form to the curriculum #597

What is the hypothesis?

In PD we have a new feedback form and we'd like it to be as accessible as possible.
We are not sure where in the curriculum it'd make most sense to add it. Add it in each module? Just once under guides? What do you think?

Why is this important?
We thought adding it to our curriculum would make it easy for volunteers to see and use - making them more inclined to give feedback.

You can have it directly on the PD block if you like? A button that goes straight to the form?

We can prefill with the title of the block as well

Incidentally, the text on your form doesn't seem finished. It cuts off mid sentence after "put them"

A direct button would work yes, thanks! Haven't noticed the text issue, we'll fix it.

Could put it in the header styled as a button

Or as an icon link (I think this is less clear)
