Update DD - not implemented

Summary

• restructuring "not implemented" columns in the data dictionary
• adding comments for void / alias rows
• adding additional sections for coded content

I will squash some commits from the rebase once approved

Looks good.
I think the descriptions of the sections of the DD should be extended roughly as follows:
Death Record Section: These fields are included in a VRO's submission of a death record.
Coded Demographics Section: These fields are included in the coded demographic content sent by NCHS to VROs. In total, these are equivalent to the legacy Mortality, Race and Ethnicity (MRE) content.
Coded Cause of Death Section: These fields are included in the coded cause of death content sent by NCHS to VROs. in total, these are equivalent to the Transax (TRX) content.
Coded Industry and Occupation section: These fields are included in the coded industry and occupation content that is produced by the National Institute of Occupational Safety and Health (NIOSH) and delivered to VROs via NCHS.
