Qualification is wrong. SHould be MD, or something, not a procedure code.

Fix comment, and use Pronouncer as the Pronouncer.

FHIR-46275 - Recreated DecedentUsualWork. Since UsualWork is only used by VRDR, it was inefficient to keep UsualWork profile in VRCL.
