This helps clinical team determine risk factors for adherence and select intensive (called supportive) or regular (called “clinical) case management

Adding Level of care Assessment.
To load this form in CHT ;

• clone the project and navigate in the config folder of the project, also make sure a running CHT instance with cht-conf install
• Run cht --url=https://<username>:<password>@domain convert-app-forms upload-app-forms -- care command to upload the form in CHT ie cht --url=https://medic:password@192-168-5-70.my.local-ip.co convert-app-forms upload-app-forms -- care
Note:Be sure to replace the values <username> and <password> with the actual username and password of your test instance
