rename and release the project

[tonyfast]

• rename the project to nbconvert-a11y
• add a release process for test releases and blessed releases.

releasing

1. tag a commit with a calver tag and a draft will be published on test pypi with a draft release created.
2. take the release out of draft mode for it be sent to pypi.
