No longer recommend es6-promise due to its size and the fact it doesn't conform to Promises/A+. Fixes #415
Hopefully this will help reduce the number of invalid issues opened.

Lots of improvements; the main one being the Table of Contents and "Read this first" section that is aimed at reducing the volume of invalid issues reported on this project.
