Apache 2.0 header added to Stanford specific source code and docs update #142

As per discussion with Vivian, all source code developed by Stanford has been updated with the approved Stanford Apache 2.0 header, README and LICENSE updated to reflect these changes.

Closes #141

@jermnelson the headers were added only to the tests we wrote, but does it also have to be applied to the configuration files (e.g. eslint) and the original files that we customized? I'm looking at https://www.apache.org/licenses/LICENSE-2.0#redistribution and the definition of "Source" under https://www.apache.org/licenses/LICENSE-2.0#definitions

these changes.

I'm gonna merge this, but am wondering if we need Apache2.txt as well as LICENSE files.
