Fix receive buffer management in CCID_Receive for ICCD B #5

Commit fixes 2 problems of CCID_Receive calls with strictly constrained rx_buffer length on ICCD_B readers:

  1. Out-of-Bounds Read when 2-byte buffer is expected and *rx_length = 2. (Expected: rx_buffer[]={90 00}. Observed: rx_buffer[]={90 XX}).
  2. LIBUSB_ERROR_IO in ControlUSB when *rx_length >= 4 because length of DATA_BLOCK received from ICCD_B device is 1 byte longer then expected data length according to standard.

In which case exactly do you have a "strictly constrained rx_buffer length" issue?

Calling SCardTransmit() with VERIFY APDU (00 20 00 00 08 31 32 33 34 35 36 37 38) and *pcbRecvLength = 2 (SW1 SW2 only expected) will reproduce problem 1.
Calling SCardTransmit() with READ BINARY APDU (00 B0 00 00 04) and *pcbRecvLength = 6 (4 bytes of data and SW1 SW2 expected) will reproduce problem 2.
By "strictly constrained" I mean that the problems won't reproduce if the value of *pcbRecvLength is more than minimal.

SCardTransmit() now correctly returns SCARD_E_INSUFFICIENT_BUFFER when
pcbRecvLength is not big enough to receive the card response.

The APDU is succesfully exchanged with the card but the card response is
lost since the client application buffer is not large enough to receive
it.

Before this patch an error was reported in the CCID driver and the
application got a SCARD_E_NOT_TRANSACTED error instead.

Now the expected size is available in pcbRecvLength for the application
to adapt its buffer size and, possibly, recall SCardTransmit().

Fixed in LudovicRousseau/PCSC@8eb9ea1

The solution is to fix pcsc-lite and not the driver.

If the creation of a channel fails then call FreeChannel() instead of
IFDHCloseChannel().
The ressources are unalocated but with no access at the lower layer.

Since the creation failed the channel is not correctly created and
only part of the reader stucture is initialised.

The problem was discovered with the folowing problem:
Program received signal SIGSEGV, Segmentation fault.
