we have a lot of documentation to write up in Markdown and it is a real challenge to use VSCode as an editor. Of course I coule just use Word but then. I see that a company has developed Writage to do this in Word but it feels more sensible to augment VSCode to do this as we'll be using VSCode for everything else in a project

We try to keep VS Code lean and we think the functionality you're asking for is great for a VS Code extension. Maybe you can already find one that suits you in the VS Code Marketplace. Just in case, in a few simple steps you can get started writing your own extension. See also our issue reporting guidelines.
