prefect-dbt artifact consolidation and markdown fixes

This combines the prefect-dbt Unsuccessful and Summary artifacts into one artifact for ease of use and further development.
It also fixes a couple bugs and reformats the Markdown to look a little nicer.

Example

No change to current prebuilt task functionality. Just changes how the Markdown looks.
