EM had to insert the contents of macro.limitedContent directly into the witness content model because the macro wasn't processed correctly for the DTD. It is better to add note to macro.limitedContent, but need to investigate ramifications - what if anything else is affected, and is it ok for them to contain note?

If it's ok to add it to macro.limitedContent, then need to go back to witness and remove the elementRef that inserts note.

Council decides we can't really do this because it's too complicated given <note>'s membership in model.global. We're closing this after investigating together.
