This update clarifies the provenance and license of buffer.h,
buffer.c, chunk.h, utf8.h, utf8.c.

Apologies, I've just noticed that the utf8.* files contain tables from utf8proc (http://www.public-software-group.org/utf8proc), licensed under the MIT/X11 license. Can you update the copyright accordingly?

@jgm: Let's just move all utf8.* code as coming from UTF8proc, as the modifications we made to the unicode conversions are minimal.
