Fix bug with receiver at depth, certain ray paths were incorrectly being removed from calculations.

Allow models with no core. ND models can also be crustless, and outer-core-less, although weird models may still generate weird results. Default for tvel is to put cmb at center of earth if only one discontinuity instead of at moho.
