Metro UI Form

Few months ago I had posted a PSD freebie [http://www.cssjunction.com/freebies/metro-ui-style-contact-form-psd/] for Metro UI Contact form. I received many requests saying for the HTML/CSS version of the same. So here is the form, coded using HTML5 and CSS3.
