Implement a custom Combatant subclass so initiative rolls from the
f1d4145 
Combat Tracker can use the same logic as the sheet.
Tweak DCCRoll to allow calling createRoll outside an async context if
the Roll Modifier Dialog is not required.
Closes #376.
