The 24-hour window ending 2026-10-21 07:59 UTC looked like a day of shared boundaries shaping many otherwise separate workflows. Across 640 collected run records, the most consequential failures occurred before agents could do useful work: a scheduled activation step stopped several jobs, while a PR checkout step stopped several reviewers at once. The later smoke runs caught a different class of problem, where execution and evidence assertions failed inside the agent job.
These are observed transitions, not a single proven root cause. A successful reviewer batch followed a failed batch on a different PR, and later scheduled successes do not establish which change, if any, fixed an earlier activation failure.
Episode Highlights
Scheduled work stopped at activation. Nine unique runs in the morning slice shared the failed
activation / Restore daily AIC scan observationsstep (clusterfailure-70bb5ebb3249c28e). The affected set included Daily Go Test Parallelizer, Feature Grower, Avenger, Daily A/B Testing Advisor, and Daily Discussion Report Dispatcher. In the audited §39408730613, the agent, detection, and safe-output jobs were skipped. Daily Go Test Parallelizer later succeeded, but the available evidence does not explain the interruption. Confidence: high on the failure boundary, low on cause. One PR event multiplied a checkout failure across reviewers. PR #70585 triggered a five-workflow reviewer batch at 18:03 UTC. PR Code Quality Reviewer, Ponytail Reviewer, Design Decision Gate, Test Quality Sentinel, and Matt Pocock Skills Reviewer failed; the audited §39471461624 stopped at
agent / Checkout PR branch. The later five-reviewer batch on PR #70581 succeeded, including §39472548266. Another checkout failure appeared on PR #70775 in §39545043201. The successful batch used another branch, so it is a comparison, not a demonstrated recovery. Confidence: high on job outcomes; cause and same-PR recovery remain unknown.Smoke checks kept finding engine-specific gaps. Smoke Pi Routed failed at
Execute Pi CLIand its routing evidence assertion in §39458068950 and §39518720281. Smoke Copilot Sub Agents failed at CLI execution and sub-agent evidence assertion; Smoke Aider failed at execution and smoke evidence verification. These tests share a validation pattern, but the saved audits do not prove a shared underlying fault. Confidence: medium.Feedback Loops Across Workflows
The activation-boundary interruption loop is mixed: one restore step blocked nine scheduled workflows before agent tools could run, followed by later successful schedules. The PR review fan-out loop is degrading for affected PRs: each PR event starts five reviewers, so a checkout failure multiplies into five lost reviews. The smoke-evidence validation loop remains unresolved: scheduled tests continue to surface execution and assertion failures across engines, with no verified corrective rerun in this window.
Human Interventions That Mattered
The observable intervention points were PR events: actor
pelikhanwas recorded on PR #70585's failed reviewer fan-out and PR #70775's later checkout failure; actorlpcoxwas recorded on PR #70581's successful reviewer fan-out. Those different PR inputs changed what the reviewer fleet processed. GitHub PR, comment, and review reads were unavailable because the read CLI hit a process limit, so no merge, review comment, or human-authored fix is attributed here.Signals to Watch Next
Check whether the same PR branch can complete all five reviewers after a checkout failure; compare the checkout inputs of PRs #70585, #70581, and #70775; and watch the next scheduled AIC restore and engine smoke assertions. The run records establish where work stopped, while the missing PR conversation and detailed step output leave the reasons open.
