Safari's back and forward buttons are disabled if application cache is enabled #85

Did two workarounds for this problem:

Now the app can be used as before.

The workaround introduced for gh-85 is not correct.  It doesn't update
location.href, so that many functions which depend on location.href
became broken.

Close gh-95.
