Describe the bug (required)
Forgot password email gives a 404

Expected behavior (required)
Forgot password workflow allows the user to reset the password.

To Reproduce (required)
Steps to reproduce the behavior:

Go to livepeer.com
Enter email in the "forgot password" form
Click on the link sent in the email
You get a 404
