Local users that are also in Django can be authenticated by exec (if I can get that working)
how to also allow auth for users that are only in Synapase?

SynapseSql does not do auth.

...or does it.

The log messages for SynapseSql say Auth enabled: false, changing it to true in config breaks things. Are the docs out of date?

For authenticate synapse users you should configure this feature: https://github.com/ma1uta/ma1sd/blob/master/docs/features/authentication.md
