This fixes http://musescore.org/en/node/51511 and http://musescore.org/en/node/47356. The loop to rewrite measures on a time signature change now stops at a section break, and after stopping - whether for the section break or because fa time signature was encountered - we check all staves to see if any lack a time signature. If so, we add one (reinstating what was already in effect).
