This may have happened with an Inexhaustible Phial.

Solutions (ideally do all of these):

• Fix Prevent Oneironaut's Inexhaustible Phial from being used in the Splicing Table #40.
• Refill the table after the cast finishes instead of in extractMediaEnvironment.
• Add a loop limit (eg. 64) to refillMedia.
