Dynamic content. How to update ranges?

I'm using annotator 2.0 with a dynamic content. Just for the record I'm using medium editor js so the content its updated live on the DOM. So far the both libraries can work together, but if I add any content before the annotation it will hold its position but the range data has already changed, so I don't know how to update all the annotations after the content has been save to keep their right position.

Any help will be appreciated.
