Comments from Tim so I don't forget:

At a quick glance it looks very good, well done! A few minor comments - I don't think the initial questions let you say "I admin perfSONAR for a community", something that allowed that would be handy. For example we will soon run Grafana mesh views for some UK communities, and we've deployed a 20-node pS platform for another organisation that isn't us. Also, if it's possible to ask if people run IPv6 tests explicitly that would be interesting, or are interested in IPv6-only test environments. And finally maybe one future option/feature might be a good viewer for traceroute and its changes (very difficult!) and maybe a test for MTU changes for those interested in running jumbo frames (tracepath is there but could be better).

Suggestion: Consider changing the question about tools to focus on tests and add another "Which tools do you use to make your measurements?"
