Entry point: README + catalog.json + everything-links-to-everything (confirm)

The June 26 discussion seemed to converge on: the README is the canonical human entry point, catalog.json is required as the machine entry point, and everything links to everything (you can navigate to any file from any other).

We believe this is already the consensus — this thread is just: confirm it, and we'll write it into the spec. A 👍 is a fine answer.
