Cannot attach file to grade feedback (Review Grade)

I want to grade a deliverable and I want to attach a feedback file (doc, pdf) to the grade before I save or submit the grade. I first choose the file and the file name shows on the grading screen next to choose file. I click on Save Draft or Save and Email. When I go back to the grading screen (Review Grade), the attachment is missing (No file chosen is displayed next to Choose File.

I tested this with a team. They can see the feedback it is indeed uploaded. It just doesn't look that way from the professor side =( I'll have some teams work on this soon.
