v2.2.1 bugfix release

A few bugs have been identified since the v2.2.0 release, so it would be good to make a quick bugfix release. The bugs and fixes are being tracked in the v2.2.1 milestone and this issue is open for general release discussion. Is there anything else that's needed, but not already tagged in the milestone?

Any thoughts on including #1040 in the v2.2.1 release? It looks mostly done, although it's a feature, not a bug fix.

Other than that, there are 3 PR remaining in the milestone. That should cover all of the bugs reported since the release of v2.2.0.

There's still issue #1041 and the corresponding PR #1043, but those changes could have more far-reaching effects to consider (see the last few comments in the PR) and the bug doesn't seem critical.

Overall #1040 seems fine, but I am wary of committing any test case that needs to do a 1sec delay to work correctly (in a program that is single-threaded). I'm wondering if there is something else that's wrong.

I think this is good to go. Everything from the milestone has been merged and the changelog is up to date (2768030).
