workflow table

have a table to track user actions though a workflow.

This idea came up as a result of our discussion with Kristen Munch from
NREL who has experience designing LIMS and things like this; she has found
that workflow tables (which track individual users' progress in various
ways) are very useful, and we can probably make use of them to track issues
that our American Gut participants encounter with the process.
