Can't delete an invalid image #670

When you add an invalid image you cannot delete the resulting missing image icon.

Steps for Reproduction

Visit http://beta.quilljs.com/
Delete everything in the editor
Click on the image toolbar icon, enter {garbage} for it's value, insert into editor
Delete the missing image icon either by hitting the delete key or by select all and delete

Expected behavior: icon is deleted

Actual behavior: icon is not deleted

Platforms: Chrome Version 50.0.2661.102, Mac
