Wrong name is used in log_fatal messages #208

Related to #203 -- log_fatal messages use the customized name, rather than the actual plugin name.

Probably it should use both, a la [Plugin / Name], for more easy identification of the problem dist.ini entry.

The name is guaranteed to be unique, so there is should be no reason to show the plugin class.
