This fixes a bug where you could quickly open the application, then change the location preferences in Settings.app resulting in a crash.

This creates the view controller used for the denied state. This also introduces a change were blocks are set to nil after they are used. This fixes issues around callbacks for errors multiple times, also it helps against delegate callbacks that can happen before the blocks are set.
