Bump. I have this problem today. Once I turn on menubar-icon-app, no amount of turning it off gets NVAlt's menu to reappear.

Is there some sort of .plist editing hack I can use to fix this?

This happened to me too.
As ttscoff suggests above, you can use Command-, (the command key and the comma key) to open up the settings if you can't see the menu bar.
