Doesn't work correctly with Multi-Root Workspace #44

Relevant https://code.visualstudio.com/docs/editor/multi-root-workspaces and https://github.com/microsoft/vscode/wiki/Adopting-Multi-Root-Workspace-APIs

I have three projects open now, with three different repositories.

However this extension just work with ONE repository at the time.

I can't initialize gitflow on a second open repository, commands always refer to the first project of the list
