SWAP debug and gadget cleanup

(Anthropic) Claude caught a small edge-case bug in swap, which they have fixed. Asking them whether each and every gadget check was productive, they noted that control-bit-flip gadgets never found any error-indicating syndromes, and this is likely because this category of error is structurally protected against by the approximation technique in general, so those could be removed. As such, it's important that ccx is protected on its AND/XOR variant rather than control qubits, and Claude has added that check. (swap ancillary checks were still found to be generally productive even after the bug was fixed.) Dan updated the is_boundary_repetition_code feature to do something more basically productive, but the recommendation for default setting is to leave this feature disabled.
