Change names of the headings in §6.1 and §6.2 of conneg-by-ap to "HTTP Headers" and "URI Query String Arguments" respectively.
This is factored out as a quick editorial fix from #544 so that we can correct this one for the FPWD.

Updated headings for §7.1 and §7.2

Updated headings for §7.1 and §7.2 to reflect that one is http headers and the other is URI query string arguments (so that it doesn't seem that QSA is independent of http)
This fixes #553
