Fix ACP conversation compaction

  • Ensure ACP multi-turn prompts use compacted conversation history to prevent context cutoffs in IDE clients
  • Harden cancelled-turn cleanup for early-abort paths
  • Eliminate speculative history push to fix cancellation race
  • Guard turn commit against late cancellation and bound the current prompt token budget
