Upgrade notes

• Breaking (contract), released as a patch: DeclarationOutcome is "created" | "kept" (#66). DeclareTemplate creates a missing slug, published as version 1; a slug the tenant already has (declared or made in the dashboard) is kept and nothing is written. DeclareTemplateCategory adds a missing id and keeps an existing one. The updated / unchanged outcomes and the declaration_conflict error are gone: drop the branches that tested them.
• Call the declarations from a Hook.TENANT_BEING_PROVISIONED handler (fired by @antelopejs/dms ≥ 0.11), not from start() or DATABASE_INITIALIZED (#65).
