修复 AI 同事被删除后以新名字替换时协作规则无法修复的问题：方案预览显示替代关系，只接受 TUI 候选，应用规则后刷新在线同事的协作上下文。重写 README。

Fixes collaboration-policy repair after a colleague is replaced under a new name: the plan preview shows the replacement, only TUI candidates are inferred, and running colleagues' contexts refresh after apply. Rewrites the README.
