Add ESM support and change to jiti

• esm support
  • This project now supports both esm and cjs (closes ESM Support #97)
• jiti
  • Big thanks to @thekip for the suggestion (closes Consider using jiti for typescript loading instead of ts-node #85)

Changed

• Support versions of Node bumped to >= 16
• esbuild transpiling updated to emit compliant modern esm and cjs
• smoke-tests updated to test and load for both cjs and esm

Removed

• ts-node has been removed in favour of jiti (closes Double Typescript compilation #70 closes Lazy loading for ts-node + optional peer-dependecies #79)
  • this was a huge point of pain from the original implementation that this repository replaces
