v0.11.0: native protocols and simpler operation

This release removes legacy compatibility paths, tightens account-scoped model discovery, and streamlines CLI internals and bilingual documentation. It includes breaking changes from v0.10.0.

Before upgrading

• Use each model's native API. Messages ↔ Responses translation and static model-capability fallbacks are removed; eligibility comes from the selected account's fetched catalog.
• Replace start --claude-code / -c with setup claude. Doctor now requires /diagnostics and does not fall back to separate endpoints.
• Per-request approval is removed. Remove --manual or change legacy manual: true only after accepting unattended forwarding; authentication and Host/Origin protection remain.
• Plaintext token diagnostics are retired (/token returns 410). Remove --show-token and retired exposure settings; use doctor or diagnostics instead. Change hosted Dashboard endpoint links from /usage to /diagnostics; the server /usage API remains available.
• Usage is taken from upstream, not locally estimated. Review the native Messages document-input boundary if you send document blocks; token-count acceptance does not prove generation support.

Container: ghcr.io/jer-y/copilot-proxy:v0.11.0 (linux/amd64, linux/arm64). Review deployment and migration requirements before restarting an existing service.
