Some measures of presortedness don't handle duplicate values

The following measures of presortedness sometimes give incorrect results when values that compare equivalent are present in the collection and need to be fixed:

• exc
• ham
• max

Fixing them will probably involve more tricks, or throwing counters at the issue, which sounds a bit like a pain, but it's probably fixable anyway.
