PR Summary

We have a new client of the PowerShell extension! Azure Data Studio! It's a fork of vscode so the extension more-or-less "just worked".

They requested a README that mentions Azure Data Studio instead of Visual Studio Code so that's what this PR contains.

They'll use this README along with our vsix and host it in their package feed that Azure Data Studio's marketplace will pull from.

I'm gonna merge this in for now, @SQLvariant is going to add some examples. Feel free to leave comments here and I will address them.
