Similar to what pandoc does with --toc-depth parameter, which limits up to which level to be included in the table of contents.

@fernandezcuesta this looks good and could be useful. Unfortunately, we are in a feature freeze right now (accepting bug fixes only). Any new features will have to wait for version 3.0.

While your addition could be applied to the "md3" branch, I'm actually waiting to break the extensions out into their own repos before making any behavioral changes to the extensions. I'll leave this open, and I expect when the time comes, this will get applied. Thanks for the work on this.

As this was originally created on the master branch, I couldn't easily add a remote to my local repo to clone it and rebase. Therefore, I've opened a new PR at #686 and this is being closed in favor of that.
