• Codex MCP configuration now emits valid, marker-bounded TOML: independently rendered MCP tables stay on separate lines, environment values use TOML inline-table syntax, and rewrites preserve user content following the managed block. Incomplete or duplicate managed markers now refuse mutation rather than risking configuration loss.
• MCP install, uninstall, and grant wiring consistently honor --project-root; same-version mcp sync repairs missing project registrations and honors declared exact, caret, and tilde version constraints without silently upgrading compatible installations.
• Codex persona descriptors use the required developer_instructions field. When no project model profile is selected, Codex resolves a compatible OpenAI profile instead of inheriting the default Anthropic profile; explicit model pins retain precedence.
• MCP health reports invalid OCI bind sources even when the Docker daemon is unavailable, and harness validation now detects a missing Copilot hook registration.
• The Nexus release workflow now extracts the selected changelog section correctly for GitHub Release notes.

• Pointer-only vendor instruction files may now resolve as symlinks to canonical AGENTS.md; substantive vendor guidance remains preserved for a deliberate migration.
• Codex per-agent MCP grants are reported as advisory rather than claiming enforcement through obsolete YAML descriptors.
• Cortex deep-reference resources are mirrored to the paths referenced by composed project instructions.
