Fixes and improvements

  • Mermaid previews now refresh immediately after clicking Allow in Obsidian’s per-vault approval prompt. No need to switch files or reopen the preview.
  • Applies to both .mmd and .mermaid files while preserving Obsidian’s approval flow.
  • Pending refreshes are discarded when the preview is closed or replaced, and event listeners are cleaned up with the preview.
  • Refresh README wording and feature grouping, including clearer desktop-only labels.
  • Simplify the preview test harness to resolve a CodeQL alert about HTML filtering.

Validation: 309 automated tests passed, along with lint, type checking, and the production build. The approval flow was also verified in Obsidian.
