20260329.1-dev.1: test(api): add rate limit test harness (31 tests)

Tests validate:
- RetryPolicy delays (15/30/60/120/300s), max retries (20)
- Exponential backoff calculation with 300s cap
- Rate limit regex detection (user_model_rate_limited, etc.)
- JSON error body parsing for 200-body rate limits
- ResponsesErrorEvent rate limit detection
- RetryPolicy.execute() retry behavior for rate limits vs auth errors

Also fix pre-existing MLXProvider Swift 6 concurrency error:
- MLXModelCache now @unchecked Sendable (safe - uses only Foundation types)
