feat: release 1.3.14 with Continue This Session In...

Adds a Continue This Session In... command to the session viewer title
bar so any saved session can be continued in any installed assistant --
Copilot, Codex, Cursor, or Claude Code -- independent of where it was
captured. The picker lists only providers whose chat command is
registered in the current host, and delivery reuses the existing resume
path. Resume This Session in Chat is unchanged.

Also adds the CodeRabbit review config.
