feat: report moves, indents and outdents as changes when using getChanges #1757

This implements detecting moves (including indents and outdents) as changes when using the getBlocksChangedByTransaction utility.

To implement this, it now has to scan the whole document to know where the parents are, so this may potentially incur a slight reduction in perf
