This release properly fixes the isLoading state issue with reactive frameworks.

Bug Fix:
- Removed premature isLoading=true assignment in fetchItem
- Loading state now only set when fetch is actually needed
- Prevents isLoading flashing to true when accessing already-loaded data
- Fixes reactive getter patterns in Alpine.js, React, Vue, and Svelte

Technical Details:
The v1.0.8 fix addressed finalizeItemMeta but missed the root cause in fetchItem.
This version removes the problematic line that set isLoading before checking if
a fetch was needed, delegating all loading state management to _startItemFetch.

Result:
- isLoading only true when fetch is in progress
- No false loading states when accessing fresh data
- Coalescing works correctly with proper loading indicators
