security: Cloud Sync RLS policy fix + threat model documentation (D-036)

- schema.sql: playlists policies → devices JOIN yerine stored device_key kullanımı
  kaldırıldı. Key rotation artık playlist satırlarını orphan etmez.
  playlists.device_key kolonu nullable yapıldı (legacy, auth'ta kullanılmıyor).
- docs/cloud-sync/migration_v1.1.sql: v1.0 schema kurmuş installs için migration
- docs/DECISIONS.md: D-036 — threat model, key derivation riski, policy fix
- README.md: clone URL YOUR_USERNAME → Simulate-X
